What is Varicocele Pain and Why Does It Occur?
A varicocele is an enlargement of the veins within the scrotum, similar to varicose veins in the legs. One of the most common questions my patients ask at my clinic in Adana is why this anatomical condition causes pain. The pooling of blood in these dilated veins increases hydrostatic pressure in the testicular tissue, leading to a dull, aching, and persistent discomfort.
How to Recognize Varicocele Pain?
There are distinct characteristics that differentiate varicocele pain from other causes of scrotal pain. Based on my clinical experience treating thousands of patients in the Çukurova region, I categorize varicocele pain with the following symptoms:
- Dull, Dragging Sensation: Rather than a sharp or sudden pain, it is typically a persistent, dragging, or heavy sensation that worsens throughout the day.
- Aggravated by Standing: Prolonged standing or sitting increases blood pooling due to gravity, making the pain significantly worse.
- Relieved by Lying Down: When you lie flat on your back, venous return improves, which usually relieves or completely eliminates the pain.
- Worse After Physical Exertion: Exercise, heavy lifting, or intense physical activities often exacerbate the discomfort.
How to Treat and Relieve Varicocele Pain?
Both temporary home care methods and permanent medical treatments are available to manage and eliminate varicocele pain. As I always advise my patients coming from the Mediterranean region, identifying the root cause through a detailed exam is the first step toward effective relief.
1. Non-Surgical and Temporary Relief
If the varicocele is in its early stages or does not yet require surgery, I recommend the following conservative management strategies:
- Wearing Supportive Underwear (Athletic Supporters): Snug undergarments or jockstraps support the scrotum, reducing gravity-induced tension on the spermatic veins.
- Cold Compresses: Applying a cold pack wrapped in a cloth can temporarily constrict dilated veins and soothe discomfort.
- Rest and Elevation: Lying down with your pelvis slightly elevated at the end of the day promotes healthy blood flow.
2. The Gold Standard: Microsurgical Varicocelectomy
The only permanent solution for persistent varicocele pain is surgical intervention. In my clinical practice, I perform microsurgical varicocelectomy, which offers the highest success rates and the lowest recurrence risks. Using a high-powered microscope, we precisely ligate the diseased veins while preserving the delicate arteries and lymphatic vessels.
