Varicocele and Male Infertility: Why Microsurgery is the Gold Standard?

Varicocele is one of the most common and, fortunately, treatable causes of male infertility. Based on my years of clinical experience with patients from Adana, Çukurova, and the broader Mediterranean region, I can confidently state that accurate diagnosis and the right surgical technique can mark a turning point in a couple's journey to parenthood.

What is Varicocele?

Varicocele is the abnormal dilation of veins within the scrotum, resembling varicose veins of the leg but occurring around the testicles. This pooling of blood causes an abnormal rise in testicular temperature, accumulation of toxic metabolites, and cellular damage, leading to impaired sperm count, motility, and morphology.

What is Microsurgical Varicocelectomy?

Microsurgical varicocelectomy is modern medicine's gold standard for treating varicocele. Through a small incision in the groin, we isolate the spermatic cord and use a high-resolution surgical microscope to magnify the vessels 8 to 15 times. This allows us to precisely identify, ligate, and divide the diseased veins while sparing healthy structures.

Why Do I Choose the Microsurgical Approach?

Compared to conventional open or laparoscopic procedures, I prioritize microsurgery for several vital clinical reasons:

  • Artery Preservation: The testicular artery, which supplies blood to the testis, is clearly visualized and preserved. Accidental ligation of this artery could lead to testicular atrophy.

    • Anesthesia: Conducted under general or regional (spinal) anesthesia to ensure complete patient comfort.
    • Incision: A mini 2-3 cm cosmetic incision is made in the groin area.
    • Micro-dissection: Under the microscope, the spermatic cord is examined to identify all vascular structures.
    • Ligation: Abnormal, dilated veins are meticulously tied off and cut.
    • Closure: Layer-by-layer closure using aesthetic sutures minimizes scarring and promotes quick healing.

      1. When does sperm quality improve after surgery?

      Sperm production takes about 74 days. Therefore, we schedule the first semen analysis (semenogram) 3 months post-op. The most significant and stable improvement is usually observed between the 6th and 9th months.

      2. What is the recovery time?

      Thanks to the microsurgical technique, patients are usually discharged on the same day or the following morning. Desk jobs can be resumed within a few days, but heavy lifting and sexual activity should be avoided for 3 to 4 weeks.
